The Surgical Treatment of Slipped Capital Femoral Epiphysis

Abstract
Purpose: To evaluate the clinical results and complications of surgical treatment in slipped capital femoral
epiphysis.

Materials and Methods: We report on 18 patients (22 hips) with slipped capital femoral epiphysis who were
treated by surgical methods. There were 13 males and 5 females. The mean age at the time of surgery was 14.2 years.
Nine cases were treated by in situ screw fixation; 6 by closed reduction and screw fixation, 4 by open reduction and
screw fixation, and 3 by osteotomy of proximal femur and plate fixation. Two-screw fixation was performed in 11
cases, three-screw fixation in 4, and single screw fixation in 4.

Results: In functional results, there were 12 excellent cases, 6 good, 2 fair, and 2 poor. As for complications,
avascular necrosis developed in 3 cases (by forceful open reduction and two-screw fixation in 2 cases, and by in situ
fixation at chronic, severe degree in 1 case), chondrolysis in 1 case (one of three screws protruded out of the femoral
head), and genu valgum deformity in 1 case.

Conclusion: To minimize complications in the treatment of slipped capital femoral epiphysis, it is important to
avoid forceful reduction, and to use in situ single screw fixation.
